I have completed this application to the best of my ability and answered all questions honestly. I have read all materials provided to me and understand that by signing below, I am agreeing to follow all Rising Sun Sober Living house rules and policies. A copy of the House Rules has been provided to me as part of this application. When I am accepted to Rising Sun Sober Living, and take residency, I agree to hold harmless Black Horse Construction, Inc., dba Rising Sun Sober Living, corporate officers, property owners, independent service contractors, and all service providers from all claims, actions and liabilities.

I authorize Rising Sun Sober Living to exchange information as needed with any and all government or private parties and/or their representatives as it relates to the application process and housing status while living at Rising Sun Sober Living.

I understand and agree that all payments to Rising Sun Sober Living are non-refundable. Reasonable accommodations for people with disabilities will be provided upon request.

I understand that exchange of substance-use treatment records is governed by 42 CFR Part 2 and that a separate Release of Information (ROI) form will be required for those exchanges.* I confirm I am 18 years of age or older; I have read and agree to the Terms of Service and Privacy Policy; I consent to receive emails regarding my application, acceptance, billing, and house communications; under the federal E-SIGN Act I agree my typed name is a legally binding electronic signature; I understand that false, incomplete, or misleading information is grounds for denial or immediate discharge without refund; and I understand that submission of this application does not guarantee acceptance.I grant Rising Sun Sober Living permission to use my photograph or likeness for non-identifying marketing, educational, or community purposes. A response letter will be sent within 3 business days and will provide instructions for "Move in Day."
